Self Certification Form

Follow these instructions to complete a self-certification for an absence.

1. Navigate to the iTrent Self-Service portal
2. Click New Surveys
   
    
3. Click Self Certification Form
   
    
4. Click Your Statement
   
    
5. Click Start
   
    
6. Fill in the questions and click Save when completed
